System Navigator

We are exploring whether a ‘systems navigator’ function could effectively support young people leaving out of home care and youth detention to navigate the complexities of these systems, ultimately improving life outcomes, including housing stability and reducing the risk of homelessness.

At an Individual (client) level

• a dedicated “navigator” role that supports young people navigating through the systems.
• a consistent point of contact for the young person throughout their support journey.
• tailored support to address the specific needs and challenges of young people.

At a systemic level

• dedicated support for case managers working with young people in addressing systemic barriers to support positive life outcomes.
• links to executive levels within social services portfolios to ensure pathway for changes, actions, and resolutions to address barriers.
• accountability mechanisms to ensure stakeholders take action.
• a collaboration with relevant services, organisations, and stakeholders to improve coordination and communication within the systems
